DESCRIPTION OF INDIVIDUAL: Individual is a 41-year-old male. He has profound ID, Autism, and Cerebral Palsy. He is nonverbal and has limited understanding of verbal communication from others and relies upon others’ tone of voice, facial expressions, gestures, or known routine to follow prompts. He will smile in the presence of a caregiver but can’t always follow a verbal direction/instruction. He typically expresses himself using non-verbal cues, gestures, and pointing.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
TRANSPORTATION NEEDS: Non-Modified transportation
DIAGNOSES: Profound ID, Autism, CP
HOUSING: Lives in a congregate setting in Painesville Twp.
DAY PROGRAM/EMPLOYMENT: He attends ADS in Mentor Monday through Friday.
