DESCRIPTION OF INDIVIDUAL: Individual recently graduated from high school and is looking for vocational and adult day services. Provider may offer both services or she may attend two different programs. She enjoys swimming, going for rides in the community, music, singing, YouTube, dancing, swinging, going to the beach, going out to eat and going bowling. Individual has limited speech and specific communication style with day-to-day needs/wants (does well with social stories, schedules, lists and 2-3 word notes). Individual has a specific routine and needs consistency to ensure a smooth day. She has history of physical aggression such as punching, hitting, and pinching others. Behavioral tracking may be needed during transition/schedule change period. Individual will engage in SIB such as hitting herself in the face if her routine is disrupted or requests are not met with appropriate tailored responses. She may yell or scream and will also hit hear head against the wall/table and/or punch or headbutt holes in walls. Guardian will provide specific training to provider to ensure her needs can be met appropriately. County board behavioral supports team is actively involved in her care to provide supports, specific education and training as needed.
PROVIDER REQUIREMENTS: Provider will need advanced experience with Autism and Crisis Intervention. She will not need medication administration during program hours.
TRANSPORTATION NEEDS: Non-modified vehicle. Will need transportation to and from program(s).
