DESCRIPTION OF NEEDED SERVICES: 21-year-old male looking for an ADS program, to spend time with friends, laid back day program activities and some community social activities too walk in the park, get lunch or do arts & crafts. Part time to full-time schedule.

DESCRIPTION OF INDIVIDUAL: Individual graduated from Broadmoor in May of 2026. He is verbal and mobile. He is quiet, keeps to himself and likes to look at his Ipad, shred, do arts & crafts, take short walks when the weather is nice and go out to eat. He would enjoy a laid-back environment, small setting and to make new friends. He may get overwhelmed in a fast-paced environment or a loud environment. He may experience self-injurious behaviors with non-preferred activities or a situation. Coping strategies; needs a break, counting to 10, or reminded to take deep breaths with a verbal prompt.
PROVIDER REQUIREMENTS: Follow OISP plan.
TRANSPORTATION NEEDS: Non-modified vehicle. Will need transportation to/from day program.
DIAGNOSES: Autism
HOUSING: Lives with parents in Eastlake.
