DESCRIPTION OF INDIVIDUAL: Individual is able to communicate verbally but should be allowed extra time to respond. She likes to help others and be a good friend. She has very good manners and is polite. She does not like confrontation and tries hard to get along with others. Individual She requires puree Diet.
PROVIDER REQUIREMENTS: Individual is in a wheelchair and will need assistance with transitioning surfaces and toileting. Individual needs support during mealtime; she may only need assistance to place food items in hand but may request staff assists if she is finding it too difficult.
