DESCRIPTION OF REQUESTED SERVICES: Individual is a 26-year-old male who resides in his family home in Madison. He is seeking NMT transportation services. NMT to travel to his worksite in Mentor from his home in Madison on Mondays and Wednesdays.

DESCRIPTION OF INDIVIDUAL: This Individual is nice, polite, always willing to help, is easy going, and has a good sense of humor. Individual is verbal and communicates effectively with others although he may talk quickly at times. He may need extra time to process his thoughts before answering a question asked of him. Individual likes to be informed of any changes to his typical schedule. He is independent with his mobility. He is reliant on others to be able to access the community. He requires visual supervision in the community. Requires staff assistance to remain safe when interacting with others, he can be too trusting at times. Individual is able to make decisions in his day-to-day life.
PROVIDER REQUIREMENTS: Follow ISP services/supports.
TRANSPORTATION NEEDS: Non- modified vehicle.
DIAGNOSES: Autism and ADHD.
HOUSING: Family home in Madison.
DAY PROGRAM/EMPLOYMENT: Individual works at SILCO in Mentor and attends Spark.
