DESCRIPTION OF INDIVIDUAL: Individual is a female in her late 20s with Downs Syndrome. Individual lives by herself in a home towards the west side of Lake County. Her personal items are very important to her and she values them greatly. The Individual is soft spoken and is typically kind but can occasionally have bad days, this usually results in the harassment of peers and resistance to supervision.
PROVIDER REQUIREMENTS: ADS.
TRANSPORTATION NEEDS: Non-Modified Vehicle
DIAGNOSES: Down Syndrome
HOUSING: Lake County-West Side
DAY PROGRAM/EMPLOYMENT: Individual currently attends ADS
