DESCRIPTION of NEEDED SERVICES: Individual is in need of HPC services to help with showering (needs support to transfer from wheelchair to shower chair, he is able to bear weight), ADL’s light housekeeping chores up to 2x a weekly, 6-10 hours weekly.
SEEKING AGENCY OR INDEPENDENT PROVIDER: Either agency or independent providers are welcome.

DESCRIPTION of INDIVIDUAL: Individual is a 33-year-old male. He lives in a home with his mom (who works outside the home). He utilizes a specialized wheelchair and shower chair. He is able to provide support while transferring as he is weight bearing. Individual has a friendly small dog which he keeps in crate while providers are at his home.
PROVIDER REQUIREMENTS: The provider will deliver ISP services as determined at the initial Service meeting.
TRANSPORTATION NEEDS: None
DIAGNOSES: Cerebral Palsy, Dystonia, and Hearing Impairment.
HOUSING: Individual lives at home with his mother.
DAY PROGRAM/EMPLOYMENT: None
