DESCRIPTION of NEEDED SERVICES: Individual needs Non-Medical Transportation 5 days per week on Monday through Friday. Arrive by 9am and pick up at 2pm. Home is in Mentor; Day Program is in Cleveland on E 22nd St.

DESCRIPTION of INDIVIDUAL: 23-year-old male who is pleasant and easy-going. Non-verbal, primarily communicating via gestures. Transportation is needed from Mentor in Lake Co to Cleveland in Cuyahoga Co. Will begin attending Lifeworks on E 22nd St. Some disruptive behavior was reported previously—touching others, playing with knobs or his seatbelt, but he responds well to verbal redirection when needed and understands verbal communication. Family is aware it may be difficult to find a Provider due to the distance.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting. Day Program is in Cleveland on E 22nd St.
DIAGNOSES: Autism, Epilepsy, ADHD.
HOUSING: Lives at home with parents in Mentor.
DAY PROGRAM: Monday-Friday at Lifeworks Atlas on E 22nd St in Cleveland.
SUPPORTS NEEDED: Transportation to and from Day Program.
