DESCRIPTION of NEEDED SERVICES: This fun 33- year- old male individual, with Cerebral Palsy is looking for HPC staffing in his home in Willowick. He has difficulty in transitioning and completing the tasks for toileting and bathing. His mom is his primary caregiver and is in need of assistance, as she is getting older. Male or female staff. Days/times; after day program and weekends.

DESCRIPTION of INDIVIDUAL: He is non-verbal and communicates via facial expressions, body language, & gestures. He loves to use his IPad to look up You Tube videos on weather, sports, cooking channels and car races. He loves car rides and to eat. He uses a walker and wheelchair for longer distances
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting; pertaining to toileting and bathing.
DIAGNOSES: Cerebral Palsy, Moderate Intellectual Disability
HOUSING: This individual lives with his mom in Willowick
DAY PROGRAM: He attends Willoughby Works.
SUPPORTS NEEDED: Assistance with completing toileting and bathing needs. His mom is getting older and struggling with this task. He doesn’t always want to comply with the tasks.
