DESCRIPTION of NEEDED SERVICES: Family is looking for ADLS support in their Eastlake home, Residential Respite Certified provider for up to 7 hours on Sundays; may be flexible weekend days.

DESCRIPTION of INDIVIDUAL: Individual is a 25-year-old male. He uses a power wheelchair but can transfer with support. He requires visual supervision and monitoring (due to seizure), especially while eating. He is a very happy, friendly and outgoing young man. He enjoys singing and playing on his tablet. He enjoys going out to dinner and going to parks.
PROVIDER REQUIREMENTS: This provider will deliver ISP services as determined within the ISP and family.
DIAGNOSES: Lowe’s Syndrome, NOS, Anxiety, Infantile Autism, Gastroparesis, GERD, Glaucoma, diastasis high blood pressure.
HOUSING: Individual lives with his mother and Sister in Eastlake.
SUPPORTS NEEDED: Individual’s mom/guardian would like residential respite assistance within home with ADLS, possible outside of the home attending leisure/social activities.
