DESCRIPTION OF NEEDED SERVICES: This individual is looking for HPC services 12-15 hours per week, preferably on Wednesdays, Fridays and Saturdays from approximately 5-9PM. This individual would like to go out into the community to go shopping, attend community events and activities, out to get coffee, go on walks, etc.

DESCRIPTION OF INDIVIDUAL: This individual loves “spooky” things and enjoys being around peers. This individual enjoys crafts and art, swimming, shopping, getting coffee or tea and going for walks. This individual does not have alone time and requires general supervision at home and visual supervision in the community. This individual has a history of resisting the supervision of family members. Additionally, this individual does not like to have demands placed on her and does not want a provider that treats her differently due to her diagnoses.
PROVIDER REQUIREMENTS: HPC and HPC Transportation
TRANSPORTATION NEEDS: Non-modified vehicle
DIAGNOSES: Generalized anxiety disorder, mood disorder, intellectual disability, autism and ADHD.
HOUSING: This individual lives at home in Mentor with her parents.
DAY PROGRAM/EMPLOYMENT: Neither at this time.
