DESCRIPTION of NEEDED SERVICES: Outgoing 57-year-old female is in need of HPC and HPC transportation in the community 3-4 days per week after ADS 2:45-3:00 pm for approximately 3-4 hours. She enjoys going out to dinner, shopping, going to the movies, church and support with personal errands. Staff would provide visual and verbal supervision while in the community.

DESCRIPTION of INDIVIDUAL: Individual is a kindhearted, social 56-year-old female who resides in a congregate setting in Mentor, Ohio
PROVIDE REQUIREMENTS: This provider will deliver ISP services as documented in the ISP. Staff provide visual and verbal supervision while in the community.
TRANSPORTATION REQUIREMENTS: Individual is mobile and can transport in a non-modified vehicle.
DIAGNOSES: Mild intellectual disability, generalized anxiety, depression, topical eczema, seborrhea, mild astigmatism, asthma related allergic rhinitis, hypertension, hypothyroidism.
HOUSING: Individual lives in a congregate home in Mentor with 3 other individuals.
DAY PROGRAM/EMPLOYMENT: Individual attends a day program 5 days a week in Mentor where she completes vocational habilitation tasks.
