DESCRIPTION of INDIVIDUAL: Social, kind, and energetic 50-year-old male who resides in a Wickliffe with his mom. He enjoys keeping busy and meeting new people. Currently works up to 30 hours at his community job using Laketran transportation.
PROVIDE REQUIREMENTS: Provide, including visual and verbal supervision in the community.
TRANSPORTATION REQUIREMENTS: Individual is mobile and can transport in a non-modified vehicle.
DIAGNOSES: Mild intellectual disability, Tourette’s, generalized anxiety disorder, OCD, acid reflux.
