DESCRIPTION OF NEEDED SERVICES: Individual is a 20-year-old male looking for an ADS program where he can be around peers and participate in community outings and activities. He is non-verbal and does not use a communication device but can make simple choices with gestures. He requires total assistance with toileting/changing depends, meal preparation, dressing and transportation. Individual requires auditory supervision in the home, visual supervision in the community and close and constant supervision when near streets and/or parking lots.

This individual requires total assistance with toileting/changing depends, meal preparation, dressing and transportation. Individual requires auditory supervision in the home, visual supervision in the community and close and constant supervision when near streets and/or parking lots.
PROVIDER REQUIREMENTS: DODD certified.
TRANSPORTATION NEEDS: Non-modified vehicle.
