DESCRIPTION OF INDIVIDUAL: 39-year-old female, very active in the community and enjoys recreational activities and opportunities to socialize and meet and hang out with people. Immediate need is for Respite—possibly overnight on the weekend with other female individuals who also enjoy being active and engaged in the community to give parents a break (currently providing Shared Living). Long-term she would be looking for a setting with at least one other female housemate who also is very social and active in the community. She needs minimal assistance with most ADLs, primarily verbal reminders. Staff need to also be active and engaged but also remind her of appropriate boundaries and be aware of her occasional need for support for increased anxiety, encouragement to be healthy and active. She loves animals, music, dancing, and playing games.
PROVIDER REQUIREMENTS: Medication Administration Certification, HPC-Transportation.
TRANSPORTATION NEEDS: Non-Modified Vehicle
DIAGNOSES: Moderate Intellectual Disability, Persistent Depressive Disorder, Anxiety, ADHD, Premenstrual Dysphoric Disorder.
HOUSING: Currently lives w/ parents in Mentor
DAY PROGRAM/EMPLOYMENT: Attends Day Program 5 days per week.
