DESCRIPTION of INDIVIDUAL: PROVIDER REQUIREMENTS: This person is a friendly 21-year-old male who is friendly. He resides with his uncle and brother in Mentor. The family is looking for ADL and community inclusion assistance. Client can be verbally and physically aggressive when demands are placed on him. Difficult to communicate with but once rapport is built it is easier. Assistance with ADLs depending on times of the day. Transportation to and from community activities. Hours are needed between 15-20/week.
A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
DIAGNOSES: Autism
HOUSING: He resides with his uncle in Mentor.
DAY PROGRAM: School
SUPPORTS NEEDED: HPC & Transportation
