DESCRIPTION OF INDIVIDUAL: 33-year-old male individual preparing for community work. He is a very kind individual. He is laid back and enjoys sports, movies, video games & music.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
TRANSPORTATION NEEDS: Requires non-modified vehicle. Need to pick up and drop off Monday through Wednesday.
DIAGNOSES: Cerebral Palsy, Mild ID, Generalized Anxiety, Major Depressive Disorder
HOUSING: Madison waiver home
