DESCRIPTION OF INDIVIDUAL: Client is a 27-year-old male. He is highly verbal, intelligent and funny. He is very social and laughs a lot. He is almost always in a good mood. He attends an ADS program during the day and needs assistance before and after his program. Individual has intractable epilepsy, cerebral dysgenesis, and moderate intellectual disability. He is in need of an HPC provider Monday thru Friday 5am to 8am. He also needs HPC staff from 2:45pm to 8pm Monday thru Friday.
In the AM hours, he needs HPC staff to assist Anthony with his gtube feed, administer his seizure medication, assist with bathing, brushing his teeth and dressing for ADS program in the AM. They will then see him off when NMT arrives.
In the evening he needs HPC staff to supervise him getting home from ADS services, engage in recreational activities, administer the dinner gtube feed, administer seizure medications and flush his bowels thru the cecostomy. He requires total assistance wiping the stool off his behind to prevent skin breakdown.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
TRANSPORTATION NEEDS: Non-modified transportation
DIAGNOSES: intractable epilepsy, cerebral dysgenesis, and moderate intellectual disability
HOUSING: Individual lives with his parent.
DAY PROGRAM/EMPLOYMENT: Individual attends an ADS program.
