DESCRIPTION OF INDIVIDUAL: 31-year-old male who lives at home—parent is Shared Living Provider. He’s very active and parents are looking for a “break” at home to allow him to socialize and spend time with someone else. Possible need to assist with evening ADLs due to seizure risk and to make sure he is thorough.
PROVIDER REQUIREMENTS: Potential HPC-Transportation.
TRANSPORTATION NEEDS: Non-Modified Vehicle
DIAGNOSES: Autism, Severe ID, Impulse Control, Epilepsy, Bipolar Disorder.
HOUSING: Currently lives w/ parents in Mentor
DAY PROGRAM/EMPLOYMENT: Attends Day Program 5 days per week.
