DESCRIPTION OF NEEDED SERVICES: 46-year-old female interested in attending ADS program 1 day/week to begin. She receives 24/hour auditory supervision.

DESCRIPTION OF INDIVIDUAL: This individual has Moderate ID, Attention Deficit Hyperactivity Disorder, Cyclothymic Disorder and suffers from severe hearing loss. She communicates verbally, speaking quickly and stuttering at times. She loves to be social with others. At times she may yell when frustrated but it is redirectable. She requires assistance communicating with others due to her hearing loss. She communicates best when she can read lips. She would need assistance with transportation, toileting and supervision/assistance with purchases in community. She loves to be involved and engage with others.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
