DESCRIPTION OF NEEDED SERVICES: Individual is looking for a provider to assist with transportation and general supervision during weekday events, such as Deepwood Rec and Special Olympics activities. Due to the nature of the activities, schedule is subject to change seasonally.

DESCRIPTION OF INDIVIDUAL: Individual is a 22-year-old male with ASD. He is fully ambulatory and has strong communication skills. He often prefers to speak minimally, but he can get his message across. Big sports fan.
PROVIDER REQUIREMENTS: Licensed in HPC or PDHPC, able to provide transportation to community activities.
TRANSPORTATION NEEDS: non modified vehicle.
