A background check is required for all persons seeking certification as an Independent or Agency Provider Director of Operations (DOO). All employees of an Agency Provider must also receive a background check. Background checks must be completed prior to hire and occur at the frequency defined in rule.

Independent Provider applicants: For certification and recertification purposes, background checks must be sent directly to DODD:

Applicants for employment with a Provider Agency/ICF must send their results to the destination specified by the Agency/ICF where employment is being sought.

DSP OnBOARD – Background Checks through the LCBDD

The Lake County Board’s Provider Support department offers Background Checks (BCII and FBI) for applicants of Independent Provider certification, Independent Provider recertification (as needed) and for Agency Provider/ICF DSP applicants through the new DSP OnBOARD program. Additionally, for Agency Provider DSP applicants, Provider Support will perform the registry checks.

Background checks and registry check results will be sent directly to DODD and the Agency Provider/ICF.

To be eligible to use the DSP OnBOARD program, you must either be

  1. seeking certification as an Independent Provider through the Ohio Department of Developmental Disabilities with intent to provide services to an individual(s) in Lake County, or
  2. an applicant(s) of Agency Providers/ICF Providers and must be seeking employment within Lake County to serve Lake County individuals.

In an effort to alleviate some of the financial impact of onboarding, the LCBDD will offer the background checks at a reduced rate.

What to bring to your Background Check appointment:

Independent Provider Certification / Recertification DSP Applicant for Agency / ICF Employment
  • Driver’s License / State ID
  • Social Security Card
  • Payment (cash or check/money order made out to the Lake County Board of DD / Deepwood)
  • Driver’s License / State ID
  • Social Security Card

You must tell us which Agency/ICF you are seeking employment with and the physical address in which to mail the results.

Background checks can also be obtained at a number of other locations throughout Lake County if you prefer not to use the LCBDD DSR OnBOARD Program.
