DESCRIPTION of NEEDED SERVICES: Individual would like HPC services to help with his ADL’s specifically showering 2x a week, Tuesdays and Thursdays, total 3-4 hours each day, making bed and light cleaning.

DESCRIPTION of INDIVIDUAL: Individual is a 33-year-old male who lives in Willoughby with his mother. He utilizes a specialized wheelchair and shower chair. Most transfers he is independent, however he requires physical support when transferring to shower chair.
PROVIDER REQUIREMENTS: HPC services 2x weekly
DIAGNOSES: CP, Hearing Impaired
HOUSING: Individual lives at home with mom.
DAY PROGRAM: Does not attend.
SUPPORTS NEEDED: Individual needs assistance transferring from wheelchair onto shower chair he is weight bearing, most transfers he is independent, however, showering he requires assistance.
