DESCRIPTION OF REQUESTED SERVICES: NMT. Individual needs transportation to and from her home in Painesville to her day program in Mentor. Individual attends program 5 days per week, and needs dropped off between 8:30-9:00 and picked up between 2:15-2:45. Individual has limited Mobility and would need a van that is low to the ground.
