DESCRIPTION OF REQUESTED SERVICES: Looking for an HPC provider to assist with the morning routine for a 9-year-old male. The hours needed are 6AM to 9AM, Monday through Friday.

DESCRIPTION OF INDIVIDUAL: The individual is a 9-year-old male. Individual lives with his adopted mom and siblings. Individual is wheelchair bound and requires total support navigating his chair. Individual requires total support with transfers, diaper changes, and ADLS. The family has a dog in the home.
PROVIDER REQUIREMENTS: Ability to assist with manual transfers.
TRANSPORTATION NEEDS: No transportation required.
DIAGNOSES: Individual’s diagnosis include traumatic brain injury (TBI), cerebral palsy, spastic quadriplegia, vision and hearing impairment, and epilepsy.
HOUSING: Willoughby, Ohio
DAY PROGRAM/EMPLOYMENT: School age.
