DESCRIPTION OF REQUESTED SERVICES: This individual is a 31-year-old female who lives in Mentor with her parents. She is looking to attend a day program with some vocational work 5 days per week. She would need physical to total assistance with all ADLs and transfers. She utilizes a power wheelchair that she can independently operate. She also requires transportation in a modified vehicle.

DESCRIPTION OF INDIVIDUAL: She is very helpful and enjoys being social! She worked in a variety of vocational settings while in high school and enjoyed doing this. She will be looking to have some work aspects built into the day, along with opportunities to socialize with her peers.
PROVIDER REQUIREMENTS: Requires assistance in the restroom.

DIAGNOSES: Spastic quadriplegic cerebral palsy
DAY PROGRAM/EMPLOYMENT: She attends a day program in Mentor.
