DESCRIPTION OF INDIVIDUAL: Individual likes to participate in crafts. Individual likes spending time with others, she prefers being around. Individual is very kind to those she cares about. Individual may become possessive over rooms/items and not allow others to use them, individual will verbally argue with others who try to use the space/object. Individual has only had 1 incident of striking another in the past twelve months (over preferred seating). Individual is redirected best when approached by preferred staff. The team has found other strategies for direction that are successful. Individual prefers routine and structure.
PROVIDER REQUIREMENTS: Must meet all requirements listed in the OISP.
TRANSPORTATION NEEDS: non-modified vehicle.
DIAGNOSES: Down Syndrome, OCD, and Depression
HOUSING: Individual lives by herself in Kirtland.
DAY PROGRAM/EMPLOYMENT: Individual currently attends ADS program 5x peer week but is looking for a change.
