DESCRIPTION OF INDIVIDUAL: Individual is an 83-year-old man, who is in a wheelchair. Currently has a pureed diet.
PROVIDER REQUIREMENTS: Med cert 1, 2, 3. 2 staffing when individual is at home to provide total assistance with all ADL’s. Requires a Hoyer lift and total assistance with all transfers.
TRANSPORTATION NEEDS: Modified, wheelchair accessible vehicle
DIAGNOSES: Severe DD, Seizure Disorder, Degenerative Arthritis
HOUSING: Waiver home
DAY PROGRAM/EMPLOYMENT: Currently in a skilled nursing facility
