DESCRIPTION of NEEDED SERVICES: HPC, Transportation, Community Outings. Looking for services 2-3 times a week from 2:00pm – 6:00/7:00pm, with one day being Wednesday (no Mondays), others are flexible, and a Saturday or Sunday when can be arranged, won’t be every weekend.

DESCRIPTION of INDIVIDUAL: The individual is a 24-year-old male who enjoys following sports teams, summer camps, eating out, and going to Round One along with other similar activities. He is verbal and independent in mobility, can use the microwave, and complete personal hygiene tasks. He can make purchases up to $50 on his own and needs verbal reminders with some assistance with routine chores.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
DIAGNOSES: Autism and Anxiety
HOUSING: The individual lives in the family home in Mentor with his mother
DAY PROGRAM: Spark ADS 5 days per week
SUPPORTS NEEDED: Visual supervision in the home and community. Assistance in finding activities and community outings he enjoys. Increase independence in daily living tasks such as routine cleaning and meal preparation. Assistance in learning appropriate social boundaries and skills along with identifying the risks of strangers to help with both social environments and future community employment. Increase independence in advocating for himself. Assistance with transportation for any outings. The proposed schedule is every Wednesday from 2:00-6:00 or 7:00pm and one other weekday evening (except Monday) for the same period. A weekend day when can be arranged.
