DESCRIPTION OF INDIVIDUAL: Individual is a 30-year-old male who lives in his family home in Willowick, OH. Individual is medically complex, requiring 24-hour care. He is wheelchair dependent and requires total assistance to navigate through his home and the community. He is in need of vehicle modification for safe transportation that includes a rear entry manual conversion ramp within a Chrysler Pacifica with tie down and seatbelt system.

DIAGNOSES: Lowe’s Syndrome, Osteoporosis (2/2/2022 increase risk of fractures), Intellectual Disability, Muscle/Ligament Disorder, Primary Open Angle Glaucoma, Amino-Acid Transport, Calculus of the Kidney, Seizures (grand mal, partial, and drop seizures), Scoliosis, Bone & Cartilage Disorder, Artificial Valve in both eyes, Blind, Tactile Defensive, Spastic Quadriplegic, Chronic kidney disease stage 4, Fracture right tibia, s/p lithotripsy, right hip dislocate, constipation.
