DESCRIPTION of NEEDED SERVICES: Individual is in need of Residential Respite (father is a shared living provider), after day program, while father is at work. Monday-Friday 2:00pm-5:00pm (flexible) and (Saturdays-flexible begin in March). Staff to provide general supervision in the home. Staff would spend time with individuals, individual loves going into the community, help prepare meals, help with minimal hygiene tasks (as needed).
SEEKING AGENCY OR INDPENDENT PROVIDER: Family is seeking a Residential Respite Independent Provider.

DESCRIPTION of INDIVIDUAL: Individual is a sweet 36-year-old male who resides with his father in Mentor. He has no medical needs, (rare seizures) or equipment. Enjoys going into the community.
PROVIDE REQUIREMENTS: This provider will deliver ISP services as documented in the ISP, to a mild-mannered 35-year-old male. Staff will monitor for seizures(rare) and follow seizure protocol
TRANSPORTATION REQUIREMENTS: Individual is mobile and can transport in a non-modified vehicle.
DIAGNOSES: Autism, moderate ID, epilepsy, depression, anxiety.
HOUSING: Individual lives in his family home with his father.
