DESCRIPTION OF REQUESTED SERVICES: This is a request for a 21-year-old male seeking ADS services and NMT supports Monday-Friday. Individual would need total assistance with ADL’s, meal preparation, navigation over steps and curbs due to visual impairment and assistance with communication devices.

DESCRIPTION OF INDIVIDUAL: This individual enjoys spending time watching his shows, listening to music on an iPad, and going for Car rides out in the community. He is non-verbal and communicates through body language, gestures, vocalizations, and a speech generative device. He enjoys spending time in the car and going for walks in his adult push chair and riding his adaptive bicycle when available. Individual does have a history of aggression and self-injury throughout his day. Individual is in the process of obtaining a service animal for seizure alert detection that would attend the program with him upon completion of training.
PROVIDER REQUIREMENTS: List of medication administration certification requirements 1,2, 3, or health related tasks to be completed by staff. List if provider must be trained in crisis prevention and intervention techniques.
TRANSPORTATION NEEDS: non-modified vehicle.
DIAGNOSES: Global Developmental delays, ataxia, cerebral palsy, visual impairment, and seizure disorder
HOUSING: This individual currently by himself in a condo and receives 24-hour support from staffing.
DAY PROGRAM/EMPLOYMENT: Individual currently attends school Monday-Friday and is expected to graduate in May of 2026.
