DESCRIPTION OF REQUESTED SERVICES: ADS and NMT up to five days a week. He would like a smaller day program with others who also like to be active and go out into the community.

DESCRIPTION OF INDIVIDUAL: He is very friendly and has a great sense of humor. Family is very important. He also likes dogs and would maybe one day like having a dog of his own. He currently enjoys going to his local senior center. He stays in contact with family and friends via his flip phone, IPad and FaceTime with assistance. At times he may experience back pain, causing him to sometimes be short tempered. Communication is very important to him.
PROVIDER REQUIREMENTS: He needs assistance with meal preparation (he is working on being able to use a microwave). He is unable to administer his own medication.
TRANSPORTATION NEEDS: Non-modified vehicle.
DIAGNOSES: Moderate ID, Spinal Stenosis of lumbar, Degenerative joint disease of the lumbar spine
HOUSING: He lives with his sister and brother-in-law in Madison, Ohio.
DAY PROGRAM/EMPLOYMENT: He is not interested in community employment at this time.
