DESCRIPTION OF REQUESTED SERVICES: This individual is an 8-year-old Male who resides in Fairport Harbor.  The individual’s family is looking for HPC services 3 days a week (Mondays, Wednesdays and Sundays) to increase the individual’s engagement in community activities as well as assist with transportation to/from speech therapy on Mondays. Mondays and Wednesdays to start at 3:15pm to ensure the individual safely gets off the bus (end time is to be determined), Sundays 11am-5pm.

SEEKING AGENCY OR INDPENDENT PROVIDER: Open to either independent or agency providers.

FUNDING SOURCE: Level 1

DESCRIPTION OF INDIVIDUAL: Individual has limited verbal skills and uses simple words usually in 2–3-word sentences. Individual can get frustrated when he is not able to express the words he is thinking. Individual is proud when he communicates with others, and they understand. Individual will smile when he is understood and will jump up/down. Individual is independent with mobility and is very active. Individual has no awareness of danger and will elope in public, school and community. Individual requires auditory supervision while in the home, close and constant supervision when in the community. Individual is terrified of dogs and other animals. Individual enjoys playing on his tablet, taking pictures, using his art supplies, water, music, exploring outside and going to the playground.

Initial services can occur in the home while provider/youth become familiar with one another. Parent will be present in the home during this transition period. Parent will also assist/ be present for initial community outings with provider/ youth.

PROVIDER REQUIREMENTS: Training in Crisis Prevention and Intervention techniques is preferred but not required.

TRANSPORTATION NEEDS: non modified vehicle.

DIAGNOSES:  Autism

HOUSING: Individual resides in his family home. Services will occur both in the home and in the community.

DAY PROGRAM/EMPLOYMENT: Individual is of school age.
