DESCRIPTION OF REQUESTED SERVICES: Individual is looking for NMT to and from their day program. This individual lives in Willoughby and attends a day program in Euclid on Mondays, Wednesdays and Fridays. This individual would need picked up in Willoughby around 8:15AM and then would need picked up at day program in Euclid at 2PM.

DESCRIPTION OF INDIVIDUAL: This individual utilizes a wheelchair and requires a wheelchair accessible vehicle. This individual does not have any alone time in the community and requires visual supervision when in the community. Individual wears hearing aids and may ask others to repeat themselves if unable to hear well. This individual enjoys being around and socializing with others. Individual does not mind being transported and is comfortable using wheelchair independently.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
TRANSPORTATION NEEDS: This individual needs a modified vehicle with a wheelchair lift
DIAGNOSES: Individual is diagnosed with diagnosed with Mild Intellectual Disability, Hearing Loss, Right Knee Weakness, Cataracts, seizure disorder (seizures have been controlled by medications for years), Osteoporosis, Anxiety, Hypothyroidism, Constipation, Hypertension, Athlete’s foot and has a history of Gastrointestinal issues and Arthritis.
HOUSING: Individual lives in Willoughby in an apartment
DAY PROGRAM/EMPLOYMENT: Individual attends a day program on Mondays, Wednesdays and Fridays in Euclid from April to December. Individual typically does not attend January to March and would not require transportation during these months.
