Looking for HPC provider to assist with running errands and socializing with a 60-year-old male. Mostly late afternoon work during the week or at any time on weekends. Flexible hours.

DESCRIPTION OF INDIVIDUAL: Individual is a 60-year-old male. He lives with his brother/guardian. Highly independent, mobile and verbal, but needs assistance with shopping, running errands, and socializing. Individual has down syndrome. He loves watching WrestleMania, car races on TV, and playing with his box cars at home. Individual attends a day program and only needs some assistance in the late afternoon and/or some weekend hours.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
TRANSPORTATION NEEDS: Non-modified transportation
DIAGNOSES: Down Syndrome
HOUSING: Individual lives with Brother/Guardian in Madison
DAY PROGRAM/EMPLOYMENT: Individual attends a day program
