DESCRIPTION OF REQUESTED SERVICES: A 32-year-old female living in Wickliffe is looking for a new HPC provider.
Schedule: Tuesday & Thursday 10:00-3:00, Saturday 11:30-3:00
ADLs: Individual needs assistance with household tasks; dishes, laundry, vacuuming/sweeping/mopping, cleaning kitchen/bathroom, &taking out trash. Individual needs assistance with personal care; transfers, getting in/out of shower, meal prep and grocery shopping.
Community: Individual needs assistance with transportation to and from frequent medical appointments. Individual would also like to increase amount of time spent in the community, interested in Deepwood recreation events.
SEEKING AGENCY OR INDEPENDENT PROVIDER: Independent

DESCRIPTION OF INDIVIDUAL: Individual has a friendly demeanor and loves to talk. Individual uses a wheelchair but is able to walk with the use of crutches or a walker. Individual smokes in her home and has a cat. Individual lives with her partner. Individual can become frustrated at times but is not aggressive.
PROVIDER REQUIREMENTS: Individual prefers female providers that can provide HPC and HPC miles.
TRANSPORTATION NEEDS: Individual uses a wheelchair but can transfer into a seat, wheelchair is small/wheels come off and can fit in most cars.
DIAGNOSES: Attention Deficit Hyperactivity Disorder (ADHD); Anxiety; Depression, Intermittent explosive disorder, CP-spastic diplegia, Seizure disorder of the absence type (controlled by medication), brain cyst, secondary Raynaud’s disease, Arthritis in toe on the right foot, GERD, Extropia, Asthma, Constipation, Carpal Tunnel
HOUSING: Individual is looking to receive HPC services in her home and in the community.
DAY PROGRAM/EMPLOYMENT: Individual attends day program 3 days a week
