DESCRIPTION OF REQUESTED SERVICES: 32-year-old female is looking for a NMT driver to take her to and from work Monday, Tuesday and Wednesday. Preferably a female. Pick up time would be 6:30 am in Concord and drop-off would be at Laketran in Painesville. She would need to be picked up from work at 1:30 at Laketran.

PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
TRANSPORTATION NEEDS: non-modified vehicle.
DIAGNOSES: Mild ID, Developmental Delay, Anxiety, Mild CP, and Hydrocephalus secondary to Meningitis with VP Shunt, Legally Blind L eye, Near & Far Sighted
HOUSING: Lives with her mom in Concord.
DAY PROGRAM/EMPLOYMENT: Works at Laketran Fleet
