DIAGNOSES: Mild intellectual disability, cerebral palsy, scoliosis/kyphosis, hydrocele, H/O bronchial asthma, amblyopia OD, high blood pressure, astigmatism, hypertension.
HOUSING: Apartment
DAY PROGRAM/EMPLOYMENT: Individual works M-Th he would need to be dropped off at 11:00am.
