DESCRIPTION OF REQUESTED SERVICES: This individual is a 55-year-old female in search of an HPC provider for morning and evening hours, 7 days per week. She has a wheelchair and is need of total support for all ADLs, including transfers using track lift, positioning, general household cleaning and laundry, care for her pets (2 service animals) including feeding, picking up after them, vet visits, etc, catheter bag emptying/care, glucose monitoring, meal preparation, community integration w/ physical assistance for purchases, clothing preparation, and other discretionary requests. Transportation for all appointments and community activities in a modified vehicle. Typical schedule is Sunday 730-9am, 3-10pm; Monday 7-9am; Tuesday-Thursday 7-9am, 3-10pm; Friday 7-9am; Saturday 9am-5pm, 6-10pm.
SEEKING AGENCY OR INDEPENDENT PROVIDER: Agency preferred due to her care needs, but is willing to interview independent providers to help fill schedule.

DESCRIPTION OF INDIVIDUAL: This individual is very bright and capable and is a good advocate for herself. She enjoys her independence and living alone in the home she owns. She loves volunteering and may ask you to help. She’s very sociable and enjoys getting out into the community.
PROVIDER REQUIREMENTS: Medication certification 1.
TRANSPORTATION NEEDS: Modified vehicle.
DIAGNOSES: Cerebral Palsy, asthma, scoliosis, IBS, type 2 diabetes.
HOUSING: She currently lives in her own home in Madison.
