DESCRIPTION of NEEDED SERVICES: HPC services in the individual’s home in Mentor, Ohio M-F 3pm-6pm, and then beginning in May, M-F 2pm-6pm

DESCRIPTION of INDIVIDUAL: Individual is a happy male who is mobile. He requires 24- hour supervision, as he is an elope risk. He needs physical assistance with all ADL’s.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
DIAGNOSES: Tetrasomy 5p, Developmental Delay, Depth Perception issues, Seizure Disorder
HOUSING: Lives with parents in a house in Mentor, Ohio
SUPPORTS NEEDED: Assistance with ADL’s and cooking. Also going on outings in the community.
