DESCRIPTION of INDIVIDUAL: This 37-year-old female is verbal. However, individual has a speech impediment that may make it hard for others to understand her, especially if she is talking fast. She is articulate when speaking. She loves playing sports, such as basketball, and being involved in Special Olympics. Likes when people use humor and are easy-going. She likes earning money and going on outings in the community. Individual has history of stealing food and money from peers. Will also ask peers to bring them food or money.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
DIAGNOSES: Mild Intellectual Disability, Irregular heartbeat, Anxiety, Bipolar disorder II, Depression, Gingivitis, Onchymycosis, Plantar warts, Asthma, Arthralgia, Seasonal allergies Shortness of breath, Binge eating disorder; Impulse control disorder, History of constipation; Pre-diabetes

SUPPORTS NEEDED: Encouragement to use self-calming techniques when needed, which include deep breathing, or taking space to herself. When having difficulty with peers, help by talking through issues and encourage her to make good choices. Individual requires visual supervision in the community. Encouragement to eat healthy and exercise as a result of pre-diabetic diagnosis.
