DESCRIPTION OF REQUESTED SERVICES: Individual is looking for a day program or a vocational habilitation program to attend 2-3 days per week, preferably in Mentor or Willoughby area.

DESCRIPTION OF INDIVIDUAL: Individual enjoys crafts, coloring and swimming. Individual also likes to watch movies and enjoys spending time with family members but has few opportunities to socialize with others and is interested in community outings as well. This individual is verbal and is able to communicate wants/needs well.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
TRANSPORTATION NEEDS: Modified vehicle with wheelchair lift
DIAGNOSES: Individual is diagnosed with mild intellectual disabilities, asthma, anxiety and adjustment disorder. Individual uses a walker indoors and a wheelchair when outside/being transported.
HOUSING: Individual lives in Eastlake
DAY PROGRAM/EMPLOYMENT: Individual would like to attend a day program 2-3 days per week.
