Looking for an ADS/Voc Hab provider for a 23-year-old male.

DESCRIPTION OF INDIVIDUAL: Individual is a 23-year-old male who lives in Eastlake. His diagnosis includes Autism, Schizophrenia, and ADHD. He needs near constant supervision and is prone to fits of anger if triggers are ignored by staff. Most of his behaviors are well controlled with medication. Typically, he is a fun, friendly, and outgoing individual. He has worked in the community with OOD unsuccessfully and is in need of a smaller more controlled environment.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
TRANSPORTATION NEEDS: Non-Modified transportation
DIAGNOSES: Autism, Schizophrenia, and ADHD.
HOUSING: Individual lives with adoptive parents in Eastlake.
DAY PROGRAM/EMPLOYMENT: TBD- new to waiver
