DESCRIPTION of NEEDED SERVICES: 39-year-old female looking for NMT to her job at Ballentine’s (Downtown Willoughby). She lives in Willowick and needs to be there by 8AM every Wednesday.

DESCRIPTION of INDIVIDUAL: This individual is verbal. She is able to speak clearly and directly and be understood by those familiar or unfamiliar with her. She is shy until you get to know her. She likes to talk about her cat and her dance team. She loves going to Dave & Busters, traveling, taking photos and going to the craft and hobby store. She is involved in Special Olympics, loves to dance & enjoys seeing her friends and family. She is independent with most tasks with some verbal prompting but is very proud of her increase in work hours.
PROVIDER REQUIREMENTS: NMT Certified and availability Wednesday mornings
DIAGNOSES: Autism, Mild ID
HOUSING: Currently living with her aunt in Willowick
DAY PROGRAM: Willoughby Branch Deepwood Industries.
SUPPORTS NEEDED: NMT only- no mobility issues or concerns. Likes consistency with pick up and drop off times so she knows what to expect.
