DESCRIPTION OF REQUESTED SERVICES: Individual is seeking NMT provider for trips between their home in Madison and their workplace in Mentor MWF. Drop off to work would need to be by 3:30pm, with pick up at 6:30pm to return home.

PROVIDER REQUIREMENTS: Able to provide NMT transportation.
TRANSPORTATION NEEDS: Non-modified vehicle.
DIAGNOSES: Moderate ID, MDD, Anxiety
HOUSING: Lives in Madison
DAY PROGRAM/EMPLOYMENT: Attends VH program
