DESCRIPTION OF REQUESTED SERVICES: 73-year-old female looking for a wheelchair accessible residential respite for a week while her family member has surgery around April or May. There is no time frame set currently. Individual has left sided weakness and is unable to transfer independently. Individual would need assistance with meals (cutting food), dressing, bathing, transportation.

DESCRIPTION OF INDIVIDUAL: Individual needs assistance with hygiene tasks (bathing, toileting, cutting up food) due to left sided weakness. She uses a wheelchair when out in the community or day program but will use a walker while at home. She requires assistance to due balance. She requires 24/7 supervision due to mobility and assistance with transferring.
PROVIDER REQUIREMENTS: Residential Respite, Medication Administration
TRANSPORTATION NEEDS: Non-modified
DIAGNOSES: Individual is diagnosed with left sided Paralysis epilepsy, grand mal seizures, Mild Intellectual Disability. She wears a brace on her left foot. She has not had a seizure in over 20+ years. She has a VNS located on her right side. She carries her VNS magnet in her bag that she takes with her to the day program.
HOUSING: Looking for residential respite for a week or two. Currently living with family in Painesville.
