DESCRIPTION OF REQUESTED SERVICES: Individual is looking to have a stairlift added to the staircase going upstairs. The individual currently cannot access the bathtub/shower on the second level.

DESCRIPTION OF INDIVIDUAL: The individual is a 22-year-old male who ambulates by wheelchair. The individual enjoys going outside, going for walks, playing with his toys, listening to music.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
TRANSPORTATION NEEDS: Modified vehicle needed.
DIAGNOSES: Cody has Cerebral Palsy, has a ventriculoarterial (VA) shunt from his brain to his heart (to allow the cerebral fluid to flow), prone to seizures (average twice/year, typically grand mal).
HOUSING: Cody lives with his mother in Mentor, Ohio.
DAY PROGRAM/EMPLOYMENT: Does not attend.
