DESCRIPTION OF REQUESTED SERVICES: The individual is a 7-year-old male who is wheelchair dependent. He lives in a small home with his parents and siblings. He is unable to access his current bathroom with his wheelchair due to the doorway being too narrow. So a parent carries him to a shower chair placed in the bathtub. This is becoming increasingly difficult and unsafe for both he and his parents. Similarly, when he is goes to bed he has to be lifted and carried into his bed. A lift track system could be a solution to these issues. Also, he would benefit from other bathroom modifications to help him be safer and more independent as he grows.
