DESCRIPTION OF REQUESTED SERVICES: 67-year-old male in need of a weekend HPC provider needed to assist at home. Assistance with all ADLs, including assistance w/ intermittent urinary catheterization and general bathroom assistance. Some physical assistance with eating (w/ utensils). Mother is 90+ years old and not able to assist as much as previous. Stepfather is weekday HPC provider but not available on weekends. Family needs weekend relief and support. Needs physical assistance navigating stairs and difficult surfaces (will place his hand on your arm).
SEEKING AGENCY OR INDEPENDENT PROVIDER: Preference for reliable Independent Provider.

DESCRIPTION OF INDIVIDUAL: Individual is pleasant, quiet, and friendly. Ambulatory but has Parkinson’s, so tremors make some activities difficult. Needs some encouragement to verbalize his wants and needs.
